HIFU, radiofrequency and thread lifts get shopped as interchangeable ways to lift a face without surgery, and they are not: two of them work by heating tissue at different depths, one is a minor surgical procedure, and only some of them are available outside a physician’s office. Why does treatment depth decide everything?

Skin laxity does not come from the skin alone. Under it sits a fibrous layer — the SMAS, or superficial musculoaponeurotic system — and that layer is what a surgeon repositions during a facelift. Any non-surgical device is judged by one question: how deep does its energy actually reach, and can it reach the layer responsible for the sagging you are trying to address?

What does HIFU actually do?

HIFU stands for high-intensity focused ultrasound. The handpiece focuses ultrasound energy to a precise depth, creating small points of controlled thermal coagulation in the deeper tissue while leaving the skin surface intact — which is why there is no wound and no downtime. Those points trigger a healing response, and the collagen remodelling that follows is what produces gradual tightening.

Two things follow from that mechanism, and both are worth knowing before you book. First, you will not walk out lifted: the visible change accumulates over roughly two to three months as collagen rebuilds. Second, because it works by stimulating your own tissue response, results vary with age, skin quality and how much laxity you started with. It is a genuine lift in the sense of a modest, measurable improvement — not a substitute for what surgery achieves.

And what does radiofrequency do differently?

RF passes energy through the skin to heat the dermis to a target temperature, which causes existing collagen fibres to contract immediately and stimulates new collagen over the following weeks. The energy is diffuse rather than focused to a point, so it works over a broader, shallower area.

In practice that makes RF the gentler option: sessions feel like a warm massage, tolerance is high, and it suits mild early laxity, crepey texture and overall firmness. It also generally needs more sessions to show its effect than HIFU. RF is frequently combined with lifting massage as a comfort-and-circulation treatment — availability of that combination varies, so ask at your consultation rather than assuming from a price list.

The pattern worth internalising: the more immediate and dramatic the lift, the more invasive the method. Anyone offering surgical-grade results from a non-invasive device is selling you something the physics does not support.

What about thread lifts, injectables and branded devices?

Thread lifts use dissolvable sutures placed under the skin to reposition tissue. They produce an immediate result, and they are a medical procedure performed by a physician — not something a medispa can or should offer. The same applies to injectables: botulinum toxin and dermal fillers require a physician’s prescription and fall under the CMQ and OIIQ frameworks in Québec. We do not provide either, and we will refer you rather than improvise.

On branded devices: several trademarked platforms deliver focused ultrasound or RF, each licensed to the clinics that own them. If you are set on one brand by name, ask directly whether a clinic runs that specific device — a clinic offering HIFU is not automatically offering the brand you read about. What matters clinically is the technology, the depth it treats, and the training of the person holding the handpiece. Ask us what we use and we will tell you plainly.

What does a realistic result look like?

What it will not do: remove significant jowls, tighten heavily loose skin after major weight loss, or replicate a facelift. If that is your situation, a surgical consultation is the honest recommendation and we will say so at the consultation rather than sell you six sessions first. For volume loss rather than laxity, the answer is also different — and physician-administered.

Because the result builds over months rather than sessions, the package length matters less here than with treatments that work per visit — what matters is completing the course and then reassessing at the three-month mark. Lifting treatments are cosmetic, so they are not covered by RAMQ or private insurance, and members save further through our membership program.
